Invitation to Participate in Research

You are being invited to participate in this survey about health and nutrition in pregnancy. Your participation is voluntary.


Why is this study being done?

You are invited to participate in this survey to help us understand what pregnant women know about nutrition, being healthy and having healthy babies. The results of this survey will help us design programs so that allpregnant women get the information they want and need to have the healthiest pregnancy and babies possible.


Your participation involves the following

You will be asked to fill out a 15-20 minute questionnaire about you, your pregnancy, and what you know or believe about health during pregnancy. We need to know about you (your age, your background, your education, your job status, etc.) so that we can understand what different populations of pregnant women know about health during pregnancy. 


Will I be paid to participate in the study?

You will not be paid to participate in this study. However, if you complete the survey you may choose to enter a lottery to win one (1) of 10 (10) $100 gift certificates to a local grocery store. If you choose to enter the lottery, you will need to give us your contact information (name, telephone number or email).


Will there be any costs to me?



What will happen to my personal information?

You are participating in this study confidentially. We will not use your name or any information that would allow you to be identified.  As soon as you enter the study, you will be assigned a study number.  Only the study number will appear on your study materials so that your identity is kept confidential. No one but Dr. Sloboda and the research team will knowthat you participated unless you choose to tell them.The information you provide will be kept in a locked cabinet where only Dr. Sloboda will have access to it.  Information kept on a computer will be protected by a password and stored on a secure network. Once the study has been completed, the data will be kept for 10 years and then destroyed.


What if I change my mind about completing the survey?

Your participation in this study is voluntary. If you decide to complete the survey, you can decide to stop at any time, even part-way through the survey.  If you decide to stop answering questions, there will be no consequences to you. You may skip any question on the survey that you are not comfortable answering. You may choose to complete the survey online. If you do, you may skip questions that you are not comfortable answering. If you would like to withdraw after starting the online survey, you can close the survey and not complete it. In both paper and online versions, when you submit this survey, you will be provided with a reference number. Please keep this number for you records. If you wish to withdraw your questionnaire after you submit, please email Dr. Sloboda and identify your questionnaire with your reference number. You can only withdraw your questionnaire up to August 1st, 2017.

This study has been reviewed by the Hamilton integrated Health Sciences Research Ethics Board (HiREB). The HiREB is responsible for ensuring that participants are informed of the risks associated with the research, and that participants are free to decide if participation is right for them.
